Clinical Assessor at Nurture Alberton Gauteng

The Nurture Health Group requires a compassionate Clinical Case Manager who will be based at our Nurture Aurora Physical Rehabilitation Centre in Gqeberha reporting to the Rehab Centre Manager.

  • Qualified RN/EN
  • At least 3 years’ case management/case management support experience in Health Care environment.
  • Exposure to Physical Rehabilitation Case Management will be advantageous.
  • Solid understanding of physical rehabilitation processes, treatment modalities and patient needs.
  • Excellent interpersonal and communication skills to establish rapport and build strong relationships with patients, families, funders, and the healthcare team.
  • Strong organizational abilities to effectively manage multiple cases and prioritize tasks
  • Empathy, compassion, and a genuine desire to make a positive impact on the lives of patients.
  • Knowledge and understanding of all Funder managed health care protocols
  • Computer literate with strong administration and organizational skills
  • Knowledge of current coding structures in the SA Health Care Environment.
  • Knowledge of therapeutic and special care patient needs’ environments
  • Must have received or be willing to receive the COVID-19 vaccination by date of hire to be considered. Proof of vaccination required.


  • Must be able to work in fast paced environment.
  • Excellent in follow up/follow through relating to coordinating and collaborating with a multidisciplinary team of healthcare professionals to ensure comprehensive care for patients.
  • Facilitating effective communication between patients, their families, and the healthcare team to address concerns and provide relevant information to all stakeholders.
  • Interpreting patient treatment plans to unlock correct authorization and benefits available to optimize patient outcomes.
  • Excellent problem-solving skills
  • Must be able to work independently as well as in a team.
  • Eye for detail and ability to conduct internal admission and discharge audits relating to coding and billing of admission events.
  • Strong literary skills.
  • Excellent telephone etiquette
  • Forward thinking skills – identify possible challenges
  • Typing skills would be an advantage
