CASE MANAGER AT NURTURE NEWLANDS- CAPE TOWN

The Nurture Health Group requires a Case Manager who will be based at our Newlands Hospital in Cape Town reporting to the Facility Manager.

The ideal candidate for this position is an individual who is:

Candidate Requirements
Applicants’ CVs will be evaluated against the following criteria:

  • Must be registered with SANC
  • At least 3 years’ case management/case management support experience in a Hospital or Medical Practice environment.
  • Exposure to Physical Rehabilitation Case Management will be advantageous.
  • Application of clinical knowledge in the risk management of patients in terms of length of stay and level of care is essential
  • Knowledge and understanding of all funders
  • Computer literate with strong administration and organizational skills
  • Proficient in current coding structures in the SA Health Care Environment.
  • Knowledge of therapeutic and special care patient needs environments
  • Must have received or be willing to receive the COVID-19 vaccination by date of hire to be considered. Proof of vaccination required.

Candidates with previous working experience in a physical-rehabilitation setting, will be preferred.
